Las causas más comunes de Traumatismo Cráneo Encefálico son los accidentes de tránsito. El 10% resultan mortales de manera inmediata, siendo el resto en su mayoría (80%) de grado leve. Si bien los traumas leves son de baja mortalidad se vinculan a una considerable morbilidad. Resulta imposible la ocurrencia de un traumatismo craneal sin que ocurra un traumatismo cervical, por tal motivo el término Traumatismo Cráneo Cervical Leve describe mejor a ambos y serán tratados de manera conjunta.
Una de las principales causas de morbilidad de los Traumatismos Cráneo Cervicales Leves son las alteraciones otoneurológicas generadas como parte del síndrome post concusional. Estas pueden clasificarse en Primarias , si ocurren dentro de las primeras 24 horas después del trauma: Concusión Laberíntica, Vértigo Postural Paroxístico Benigno, Fistula Perilinfática y Trastornos Centrales. Las alteraciones Secundarias ocurren más allá de las 3 semanas y son la Disfunción Otolítica y el Sindrome de Meniere Tardío.
El mareo cervical es una entidad clínica controvertida, y no existen pruebas suficientemente sólidas que lo justifiquen como tal.
Los estudios otoneurologicos favorecen el diagnostico precoz de las alteraciones vestibulares post traumáticas, facilitan la instauración de un rápido tratamiento, veloz recuperación y evitan el reposo prolongado.

The main causes of Head Trauma are road accidents. Approximately 10% of these are fatal immediately and the most part of the remaining cases ( 80%) are mild . Although mild head trauma are not fatal they are associated with high morbidity. It is impossible the occurrence of head injury without cervical damage, so the term Mild Head and Neck Trauma describes them best.
Morbidity following Mild Head and Neck Trauma can be attributed to vestibular dysfunctions as part of the post-concussion syndrome. The Primary dysfunctions that affect patients within the first 24 hours after trauma are: Labyrinthine Concussion, Benign Paroxysmal Positional Vertigo, Perilymphatic Fistulae and Central Vestibular Disorders. The Secondary dysfunctions affect patients at a later date, more than 3 weeks: Otolith Disorders and Delayed Meniere Syndrome.
Cervical dizziness is a controversial clinical situation and all clinical studies on cervical dizziness are not solid enough to justify it as a clinical entity.
Otoneurological studies promote early diagnosis of vestibular disease after trauma, allows to stablish a fast treatment and recovery and avoids long term disabilities for the patient.

El sistema vestibular es muy complejo y posee una enorme capacidad de adaptación que permite mantener distintas posturas y realizar diferentes movimientos. Además presenta una gran plasticidad para recuperar funciones cuando éstas se encuentran alteradas o dañadas. La Rehabilitación Vestibular es el conjunto de ejercicios y maniobras destinadas a favorecer los mecanismos compensadores del Sistema Nervioso Central. A través del conocimiento de la fisiología, del examen físico y de los exámenes complementarios se arriba a un diagnóstico, fundamental para seleccionar las estrategias que garanticen una Rehabilitación Vestibular adecuadas para cada patología en esta área.

The Vestibular system is very complex and has the hability to adapt easily,which allows you to maintain different postures and perform different movements. It also presents a great plasticity to retrieve roles when they are altered or damaged. The Vestibular rehabilitation is the set of exercises and maneuvers that favor the compensating mechanisms of the Central Nervous System. Through knowledge of physiology, physical examination and complementary exams, a diagnosis is reached, which is the key to selecting strategies that guarantee a vestibular rehabilitation for each pathology in this area.

La cinetosis, o sensibilidad al movimiento, es el conjunto de signos y síntomas (náuseas y/o vómitos, palidez, sudoración, cefalea, mareos, etc) provocados por movimientos externos o del campo visual, de aceleraciones y desaceleraciones lineales y angulares repetitivas. La incidencia es mayor en mujeres que en hombres. La susceptibilidad es sumamente individual y muy variable. Los pacientes con historia de migraña, y particularmente de migraña vestibular, son más propensos a la sensibilidad al movimiento. La causa primaria es la estimulación desproporcionada del aparato vestibular por el movimiento pasivo, generando un desequilibrio entre las aferencias sensoriales, especialmente entre los sistemas visual y vestibular. No existe examen complementario propio para ésta condición. Existen múltiples terapéuticas tales como medidas generales (modificaciones conductuales físicas y visuales tendientes a la reducción del conflicto sensorial), medidas farmacológicas (hioscina, dimenhidrinato, metoclopramida, etc) y medidas no farmacológicas. El Mal de Debarquement, también conocido como mareo de tierra, es la persistencia de un desequilibrio (inestabilidad, giro, balanceo, etc) que ocurre al retornar a tierra después de un viaje en barco. A diferencia de la cinetosis, casi carece de componente vegetativo.

The motion sickness, or sensitivity to the movement, is the set of signs and symptoms (nauseas and/or vomits, pallor, perspiration, migraine, dizziness, etc) caused by external movements or of the field of view, of linear and angular accelerations and repetitive decelerations. The incidence is greater in women than in men. The susceptibility is extremely individual and very variable. The patients with history of migraine, and particularly of vestibular migraine, are more prone to sensitivity to the movement. The primary cause is the out of proportion stimulation of the vestibular apparatus by the passive movement, generating an imbalance between the sensorial inputs, specially between the visual and vestibular systems. Own complementary examination for this one condition does not exist. Exist multiple therapeutic such as general measures (physical and visual conducts modifications tend to the reduction of the sensorial conflict), farmacological measures (hyoscine, dimenhydrinate, metoclopramide, etc) and non farmacological measures. Debarquement Syndrome, also known as "Landsickness", it is the persistence of an imbalance (instability, turn, balance, etc) that happens when returning to earth after a boat trip. Unlike the motion sickness, almost it lacks vegetative component.

El Vértigo Visual (VV) se define como toda sintomatología vestibular provocada por la estimulación visual, generalmente secundario a la exposición a imágenes a campo lleno repetitivo/luminoso o con movimiento de entornos visuales. Acorde a lo descripto por Brandt, el vértigo visual puede ser "fisiológico" (desorientación espacial, vértigo de las alturas, acrofobia, vección lineal y rotatoria, efecto pseudo-Coriolis, efecto pseudo Purkinje, mareo por estímulo optocinético) o "patológico" (por enfermedad o disfunción). Tanto lesiones centrales como periféricas han sido descriptas en la génesis de los vértigos visuales. Según Bronstein, el Vértigo visual está presente en muchos pacientes con antecedentes de un trastorno vestibular periférico, especialmente los que son "dependientes visuales", es decir, aquellos sujetos que utilizan la visión preferentemente para el control postural y la orientación espacial. La mayoría de los pacientes con VV tienen poca o ninguna anormalidad en las pruebas vestibulares convencionales. Existen varios diagnósticos diferenciales tales como la migraña vestibular, traumatismos cráneo-encefálicos, post-síndrome de conmoción, síndrome del latigazo cervical, y trastornos psicológicos/psiquiátricos (incluyendo ataques de pánico). El tratamiento para el vértigo visual implica la Rehabilitación Vestibular como método de compensación. La Oscilopsia es la ilusión de oscilación de los alrededores visuales. Se diferencia del vértigo visual en que ésta sólo se presenta durante movimientos de la cabeza (o de todo el cuerpo). Dentro de sus causas, la más frecuente es el defecto bilateral en el reflejo vestíbulo-ocular VOR.

The Visual Vertigo (VV) is defined as all vestibular symptomatology caused by the visual stimulation, generally secondary to the exposure to luminous or repetitive full-field images or with movement of visual surroundings. Agreed to what is described by Brandt, the visual vertigo can be "physiological" (spatial disorientation, vertigo of the heights, acrofobia, linear vection and rotatory, pseudo-Coriolis effect, pseudo-Purkinje effect, dizziness by optokinetic stimuli) or "pathological" (by disease or dysfunction). As much central as peripheral injuries have been described to be the origin of the visual vertigos. According to Bronstein, the visual Vertigo is present in many patients with antecedents of a peripheral vestibular disorder, specially those that are "visually dependent", that is to say, those subjects that preferably use the vision for the postural control and the spatial orientation. Most of the patients with VV they have little or no abnormality in the conventional vestibular tests. Several differentials diagnoses exist such as vestibular migraine, skull-encephalic traumatisms, postsyndrome of commotion, neck whiplash, and psychiatric/ psychological disorders (including panic attacks). The treatment for the visual vertigo implies the Vestibular Rehabilitation like compensation method. The Oscillopsia is the illusion of oscillation of the visual environs. The difference of the visual vertigo in which this one only appears during movements of the head (or of all the body). Within its causes, most frequent it is the bilateral defect in the vestibular-ocular reflex VOR.

Describimos una serie de niños que comparten características electroclínicas de una epilepsia focal con buena respuesta a fármacos y manifestaciones vestibulares predominantes, sugiriendo un origen témporo-parietal. Se realizó una revisión de carácter retrospectivo de historias clínicas por un período de 2 años; los pacientes cumplieron los siguientes criterios: presencia de síntomas vestibulares predominantes o exclusivos, rango de edad entre 5 y 18 años y resonancia magnética de cerebro con angioresonancia normal. Siete pacientes fueron finalmente seleccionados. Los síntomas comenzaron abruptamente como vértigo rotatorio de segundos de duración. Las características predominantes en el Electroencefalograma interictal fueron anormalidades en áreas temporo-parietales. La demora diagnóstica fue de un promedio de 3 meses. Todos los pacientes presentaron excelente respuesta a las drogas antiepilépticas utilizadas.

We describe a serie of children who share electroclinical characteristics of a focal pharmacoresponsive epilepsy manifesting as prominent vestibular disturbances, suggesting a temporo-parietal ictal origin. We retrospectively reviewed a database of patients over a 2-year period following these criteria: presence of prominence or exclusive paroxysmal vestibular symptoms, age range between 5-18 years-old and normal magnetic resonance angiography (MRA). Seven patients were finally selected. The symptom began abruptly as a rotational vertigo during several seconds. The predominant feature on interictal scalp EEG were abnormalities over the temporo-parietal areas. The diagnosis was usually delayed with an average of 3 months. All patients responded well to antiepileptic medication.

En los últimos años, los conceptos sobre la clínica, etiología, diagnóstico, y tratamiento de la isquemia del territorio de la circulación posterior han cambiado sustancialmente.
Muchos casos de enfermedad vertebrobasilar son subdiagnosticados, en general los pacientes presentan un conjunto de síntomas y signos que dependen del área isquémica. El diagnóstico se centra en un adecuado interrogatorio, el exámen neurológico completo y considerar la edad del paciente y sus factores de riesgo cerebrovascular
Los estudios por imágenes han revolucionado los conceptos sobre el tratamiento y el pronóstico.
Es importante en la evaluación del otorrinolaringólogo pesquisar o descartar la patología vertebrobasilar, utilizando todos los recursos clínicos disponibles y eventual interconsulta y/o derivación temprana al especialista.

La anatomía vestibular representa el primer paso fundamental en la comprensión de la fisiología y la patología vestibular. El tamaño y la compleja conformación tridimensional hacen más difícil dicha comprensión. Si bien la anatomía no cambia, los nuevos avances en la tecnología por imágenes, como en los nuevos métodos de diagnóstico y tratamiento, hace que cambie la forma de interpretar y de ver la patología, por ello es necesario el conocimiento más detallado de la anatomía, que espero lograr ayudado por la mostración de imágenes obtenidas mediante la disección cadavérica.

The Vestibular anatomy represents the first fundamental step to understand the physiology and the vestibular pathology. The size and the complex tridimensional conformation of the inner ear, make it even more complex to understand as a whole concept. Although anatomy does not change, the new technological advances in imaging technology, as well as in diagnostic and therapeutic methods, make it different to analyze and see the inner ear, and for diagnostic purposes it needs an advanced knowledge of the anatomic details. I hope to help with this complete understanding of the inner ear, with images obtained from different cadaveric dissections.

Vértigo y mareos son síntomas frecuentes en la consulta médica de la población mayor. Se reportan en el 30% de los mayores de 65 años, llegando al 60% en los mayores de 80. Datos de la encuesta nacional de salud y nutrición para adultos en Estados Unidos demuestran que la prevalencia de disfunción vestibular es de 49,4% en el grupo etario de 60–69 años, del 68.7% entre los 70 y 79 años, llegando al 84,8% en los mayores de 80. El vértigo postural paroxístico benigno y el mareo multisensorial son las causas más frecuentes de mareos entre los adultos mayores. Su tratamiento es individualizado, se debe planear una estrategia un programa de rehabilitación vestibular, que incluya entrenamiento del equilibrio y de la fuerza muscular.

Vertigo and dizziness are common complaints in elderly population. These symptoms are reported in 30% of those older than 65 years, reaching 60% in people older than 80. Data from the national survey of health and nutrition in the United States in adults show that the prevalence of vestibular dysfunction is 49.4% in the age group of 60-69 years, of the 68.7% between 70 and 79 years, reaching 84.8% over the age of 80. Benign paroxysmal positional vertigo and multi-sensory dizziness are the most frequent causes of dizziness among older adults. Treatment is individualized, and includes a program of vestibular rehabilitation, with training of muscle strength and balance.

La Vestibulopatía bilateral es un raro desorden del laberinto periférico o del octavo par debido a diferentes etiologías. Los síntomas principales son la oscilopsia durante la marcha o con los movimientos de la cabeza y la inestabilidad principalmente en la oscuridad. La rehabilitación vestibular puede mejorar la estabilidad postural y disminuir la sensación de desequilibrio en muchos pacientes habilitándolos a volver a su vida normal.

The bilateral vestibulopathy is a rare disorder of the peripheral labyrinth or the eight nerve which has various aetiologies. The key symptoms are oscillopsia during locomotion or head movements and unsteadiness particulary in the dark. Vestibular rehabilitation can improve postural stability and decrease the sense of disequilibrium in many patients, enabling them to resume a more normal life.

Se define como Traumatismo Craneoencefálico Leve a aquel que posee una puntuación en la escala d Glasgow entre 13 a 15, amnesia post traumática menor a las 24 hs de duración y pérdida de conciencia menor a los 20 minutos de duración. Gran cantidad de estudios acerca de traumatismos craneoencefálicos sugieren que síntomas psicológicos y vestibulares son algunas de las secuelas más frecuentemente referidas luego de traumatismos leves.
Numerosos pacientes que padecen alteraciones del equilibrio luego de un traumatismo craneoencefálico leve son derivados a nuestros consultorios y la videonistagmografía es el primer estudio que se les realiza. Por tal motivo consideramos de interés determinar los síntomas vestibulares prevalentes y evaluar la VNG como herramieta de diagnóstico rápida y precisa.

Mild head trauma is defined as a Glasgow Coma Scale score of 13 to 15, posttraumatic amnesia of less than 24 hours and loss of consciousness less than 20 minutes.
Several studies of the sequelae of head trauma suggest that psicologic and vestibular dysfunctions are some of the most frequently reported complications after mild head injury.
Many individuals with balance disorders after MHT are referred to our center and videonystagmography is the first study performed in this population.
We considered of interest to determine prevalent vestibular symptoms and to evaluate VNG as a fast and accurate diagnostic tool.
